Cocktail Slots Scratch Card Official Rules
1. NO PURCHASE NECESSARY TO ENTER OR WIN. American Beverage Marketers, Master of Mixes® Cocktail Slots Scratch Card Instant-Win Game ("Promotion") begins at 12:00 AM Eastern Standard Time ("EST") on Sunday, January 1, 2004 and ends on Sunday, February 15, 2005 at 12:00 PM Eastern Standard Time (“EST”) or, when the supply of all 1,000,000 Instant-Win Scratch Cards (“Game Cards”) are exhausted, whichever occurs first. Game cards will be distributed on top of randomly selected bottles of Master of Mixes®. Alternate means of entry: Mail a handwritten request and self-addressed, stamped #10 or larger business size envelope (VT residents may omit return postage) to: Cocktail Slots Scratch Card Request, P.O. Box 22392, Louisville, KY 40252. Limit: One (1) Game Card request per outer postmarked envelope. No mechanically reproduced requests allowed. Requests must be postmarked by February 1, 2005 and received by February 15, 2005. Not responsible for late, lost, misdirected, illegible, mutilated, incomplete or postage-due mail/requests.
2. HOW TO PLAY THE INSTANT-WIN GAME: Game card has a total of (3) columns. Scratch off ONLY (1) area per column. If you scratch off any portion of more than (1) scratch area per column, game card is void. If you reveal (3) Master of Mixes® logos you win Grand Prize (subject to verification). If you reveal (3) images of the same fruit, you win a prize (subject to verification, see prize chart). If you reveal (3) images of Master of Mixes® bottles you win a coupon for a Free Product. A maximum of five (5) entries may be submitted per household. You must comply with official rules to claim prize.

Total value of expected winners, based on probability: $392,379.00. Limit five (5) prizes per person/household, except for sixth prize.
4. HOW TO CLAIM A PRIZE: Grand Prize, First Prize, Second Prize Winners: Send winning game cards to American Beverage Marketers, Marketing Dept.,, 810 Progress Blvd, New Albany, IN 47150. Game cards must be sent via Certified Mail USPS, FedEx, UPS, or other traceable carrier.
Third Prize, Fourth Prize, Fifth Prize and Sixth Prize Winners: Mail a letter with your full name, address and a daytime telephone number, the original winning game piece to Scratch Card Winner, P.O. 22392, Louisville, KY 40252.
Please make a copy of your winning game piece. American Beverage Marketers is not responsible for any lost game cards because of undelivered mail. All winning claims must be postmarked by February 15, 2005 and received by February 28, 2005. Verified prize winners will be sent a check or coupon (fifth prize and sixth prize) in the mail within 6 to 8 week after receipt of winning game piece. American Beverage Marketers is not responsible for misdirected mail or the wrong address provided by prize winner.
If all tickets are not distributed, some prizes may not be awarded. Game Cards must be surrendered when claiming prize. If the winning entrant of any prize cannot be contacted; is ineligible; fails to claim the prize; or if the potential winner otherwise fails to fully comply with these official rules, he/she will forfeit that prize and the Instant-Win prize will not be awarded. Entrants grant Sponsor the right to use and publish winner's proper name and State of residence. Acceptance of a prize constitutes permission for Sponsor and its affiliates to use winner's name and likeness solely for advertising and promotional purposes as Sponsor so determines without notice or further compensation, except where prohibited by law.
5. VERIFICATION:
All Game Cards are subject to verification before declared “winning” Cards.
Winning game cards will become the sole property of Sponsor and will not be
returned. Game Cards are void and will not be accepted if they have more than
one area in whole or in part revealed in each column.
Game card will be void which are found to be mutilated, illegible, altered,
counterfeit, tampered, failed at any validation test, or obtained through
unauthorized channels, as determined by Sponsor in its sole judgment. Only the
stated number of potential prizes will be awarded. In the event of a printing
or other error resulting in more winning Game Cards than available in a given
category of prizes, a random drawing will be held among those holding winning
Game Cards (i.e., three matching symbols) to award the number of prizes in that
category stated in these official rules. Liability for defective Game Card
limited to replacement with another Game Card while supplies last.
7. GENERAL INFORMATION: This Promotion is conducted in English (which will be given its everyday ordinary meaning), and any and all disputes will be resolved in the appropriate courts in the County of Floyd in the State of Indiana, exclusively, and subject to the laws of the State of Indiana. Should there be a conflict between the laws of the State of Kentucky and any other laws, the conflict will be resolved in favor of the laws of the State of Indiana. Sponsor or MCC Printing and Promotions has the right to cancel this Promotion at any time for any reason. All federal, state, local, and other taxes on prizes are the sole responsibility of the winner.
8. ELIGIBILITY: A maximum of five (5) entries may be submitted per household. The Promotion is open only to legal residents of the 50 United States and the District of Columbia. Entrants must be 21 years of age (legal drinking age) or older at the time of entering Promotion. Employees, officers and directors of American Beverage Marketers and its subsidiaries, including American Beverage Marketers and their respective vendors, resellers, brokers, MCC Printing and Promotions, LLC., their immediate families (parent, child, sibling & spouse) and persons living in the same households as such individuals (whether related or not) are not eligible to participate in the Promotion. By entering in the Promotion, participants agree to release, discharge and hold harmless American Beverage Marketers, its distributors, brokers, resellers, MCC Printing and Promotions, LLC., their respective parents, affiliates, subsidiaries, vendors, agencies and their respective officers, directors, employees and agents, from any and all damages whether direct or indirect, which may be due to or arise out of participation in the Promotion or any portion thereof, or the acceptance, use/misuse or possession of prizes. By participating, you agree to these Official Rules and to the decisions of American Beverage Marketers whose decisions are final and binding in all respects. Void where prohibited by law, rule or regulation. All federal, state and local laws and regulations apply.
9. WINNERS LIST: To receive a list of winners, send a self-addressed stamped envelope for receipt by February 15, 2005 to: Winners List Request, P.O. Box 22392, Louisville, KY 40252. Winners lists will be mailed after all winners have been confirmed.
